Client Consultation Record
You have requested a microblading/micropigmentation treatment that involves minor breakage of the skin surface and this process may complicate some medical conditions. Please read the following information carefully and if any of these conditions apply to you, you must declare them to the practitioner on the premises and discuss these matters. Also colour durability and reflection factors will be discussed with you during this consultation.

HEALTH CONDITIONS
Skin conditions
Eczema this may make a person more prone to skin infections/ irritation.
Psoriasis or other chronic skin conditions, excluding acne and disorders of pigmentation- same complications as eczema.

Circulatory Disorders
Heart disorders some heart defects render individuals more prone to serious heart complications from any blood infections.
High/low blood pressure can cause light headedness and may be linked to other heart- circulation disorders.
Haemophilia and other bleeding disorders- as may result in poor healing.

Pregnancy
Nursing mothers treatment must not interfere with the feeding process; also, any risk of infection for them is also a potential risk for their baby
Pregnancy the immune response may be affected by pregnancy; any infection may affect an unborn child.
Other medical conditions
Epilepsy and persons who have experienced a seizure in last two years. Medication may cause side effects and poor control of the condition may result in fitting/seizure during treatment.
Diabetes long term sufferers may have circulation problems that can reduce healing properties of the skin; this can result in severe infection.
Auto immune disease or other conditions or treatments causing immune- deficiency (e.g. cancer treatments-radio/chemo therapy)- more prone to serious infections; HIV a risk factor for practitioner.
Hepatitis C may have circulation problems that can reduce healing properties of the skin; this can result in severe infection. It also represents high risk to the practitioner to get infected.
Medication- Warfarin, Antabuse, Roaccutane ;side effects may affect healing and recovery from treatment.
Allergic responses
Allergies especially nickel allergy, may result in serious skin reaction from small amount of metals sometimes present in applied products (inks etc.)
Medical history of keloid scarring
People who have previously had keloid scars, are more likely to develop one at the treated area.
Other considerations before you undergo treatment
General treatment cannot be undertaken if you are under influence of alcohol or drugs
Any other conditions any visible evidence of a cold sore or fever blister or a skin disorder, broken capillaries on the area to be treated.
Explanation
What to expect
The client is informed in detail by Artist about specific risks of eyebrow drawing treatment using microblading technique.
The following risks are specifically explained to the client:

During the treatment, despite the staff expertise and all the precautionary measures, an injury is possible. Despite the application of the most advanced and the top quality pigments, allergic reaction is possible but rare. The client is informed about this and he/she assumes liability.

During and after the treatment temporary swelling, redness and/or itching may occur. Experience tells us that these symptoms are temporary.

Depending on the skin structure after the first treatment small scabs with a loss of drawn hairs may occur and color intensity may change. In the first seven days eyebrows are up to 40% darker and 10-15% thicker. Color i.e. color reflection depends on the natural skin pigment.

The shape is determined according to the face proportions. Symmetry is determined digitally, with closed eyes because of the negative impact of facial expression.

The pigment is absorbed differently due to differences in the skin quality thus there is no warranty for the treatment success. Depending on the skin structure, it should be noted that change in the color intensity is possible and that one or more additional treatments will be required.

The minimum or maximum duration of eyebrow drawing treatment cannot be determined with certainty, nor can the warranty be given on performed treatment. The first correction is done six weeks after the treatment. For oily skin it is necessary to perform more corrections.

Permanent make-up always leads to the skin injury. Therefore. it is important to carefully and gently nurture your skin after the treatment to allow healing without complications. Inadequate care in healing phase of the skin can lead to poor results and Artist cannot be liable for it.
Before Treatment
Client are advised;

Before your brow treatment;

Do not drink coffee, alcohol, or energy drinks 24 hours before treatment.

Do not take blood thinners such as Aspirin, Niacin, Vitamin E, Ibuprofen, fish oil or vitamins 24 hours before.

No waxing 2-3 days before treatment.

Do not tan or have intense sun exposure 3 days before treatment.

If a client uses retinoid /retinol products, these should not be used at least 3 days before the treatment.

Botox and fillers on forehead, temple and eye area should be avoided 4 weeks before treatment.

No chemical peels or laser peels, dermabrasion or any other intense treatments for 2 months before treatment.

Aftercare
What to do after your treatment
For the next 7 days clients are required to pay attention to the following:

Half an hour after the end of the treatment eyebrows should be carefully wiped with the Aftercare Wipe provided, and then a thin layer of Phibrow Skin Candy should be applied. This procedure should be repeated 3- 5 times a day to prevent a scab formation. For post-treatment care use only cream provided by Artist or pure Vaseline. Please do not use any other creams except the ones provided to you in order to prevent possible infections or allergic reactions.

KEEP BROWS DRY. Disposable protective brow patches can be purchased to help with this.

Avoid applying makeup in the eyebrow region.

Avoid workouts that involve excessive perspiration, saunas, facial massage and steaming.
For the next 30 days

Avoid sunbathing, tanning beds, light therapy, chemical peels, fruits acids, microdermabrasion, creams that contain regeneration factors.

Always avoid laser treatment over treated area (fraxel laser,IPL) because they may destroy the pigment and cause burning.

Use of antibiotics and hormonal therapy can lead to faster pigment fading.

Always protect your eyebrows from the sun.

The Artist is not liable in case of improper post-treatment care.
